Improving to basic facilities such as water, sanitation and housing is a key aspect of the Reconstruction and Development Programme (RDP). Although most local authorities can identify the most undeserves areas, estimates of the extent of the need are likely to provide the most effective motivation for targeting investment and planning interventions. To this end, a survey of access to basic facilities, including housing, water supply and sanitation, was carried out by the Health Department of the Cape Metropolitan Council (CMC) for the Greater Cape Town area, excluding the city centre and other areas not serviced by the CMC, between 1994 and 1995. Population estimates for the greater metropolitan area were also obtained.

There are substantial resources available for meeting the health needs of South Africans. However, there are gross inequalities in the distribution of these resources between the public and private sectors, between levels of care, and between geographical areas. A major redistribution is required, but this will have to be managed in order to minimise disruption.

The theme of this month's "Update" is greater integration of the public and private health sectors in South Africa. It explores ways in which convergence of public and private health care in South Africa can be used to increase people's access to health care, and to ensure both greater efficiency and equity in the provision of health care. Some of the suggested mechanisms are complex, controversial and may take time to implement. Other steps may be simpler and should be introduced now - if only to prevent the situation described above happening again.
